If you are worried about safety, data or how something is being done at Eviqa Ltd, tell us. We take every concern seriously and you will not be treated unfairly for raising one.
How to raise a concern
Email info@eviqa.health with "Concern" in the subject. If you would rather speak to someone, call 01626 717587, 9am to 5pm, Monday to Friday.
If someone is in immediate danger, call 999.
If you have found a security problem in Eviqa Assure, see reporting a security issue.
Staff and contractors
You can raise a concern in confidence. You can also contact the relevant regulator directly, such as the Information Commissioner's Office for data protection or the Care Quality Commission for concerns about care.
